Which FAI version? 
I guess something is wrong with the tftp setup.
Check is user tftp can read all file in /srv/tftp/fai.

    > tftpd-hpa config:
    > # /etc/default/tftpd-hpa
    > TFTP_USERNAME="tftp"
    > TFTP_DIRECTORY="/srv/tftp/fai"
    > TFTP_ADDRESS="0.0.0.0:69"
    > TFTP_OPTIONS="-l -s"
This looks good.
