Did you use dpkg -i fai_2.... or apt-get install fai ?
Calling fai-setup should set up fai.conf in the nfsroot and also
install pxelinux.0

No symlink. Make a copy but fai-setup should do this for you.

No. fai-setup will do this.

For debugging call make-fai-nfsroot -v
which gives verbose output.
