FAI install PXE boot hangs: "add_mountroot_fail_hook not found"

Dear all,
I followed the instructions on
http://wiki.fai-project.org/wiki/Installing_Ubuntu_Linux_with_FAI to
install Ubuntu 14.04 from an Ubuntu 14.04 server. However when the
client boots via PXE, at some point it complains that in script
/scripts/init-premount/mdadm and /scripts/init-premount/lvm2 it cannot
find "add_mountroot_fail_hook". Later on the boot process loops forever
in an USB disconnnect-USB connect cycle of the USB mouse.
I'm not sure if the two issues are connected, but that's about what I
can see on the console.
fai-setup has been run as `fai-setup -vlfk`. fai-setup.log is attached.
I'm not sure what to do with the messages regarding initramfs-tools in
the fai-setup.log, either. The system is up-to-date:
# apt-get upgrade
Reading package lists... Done
Building dependency tree
Reading state information... Done
Calculating upgrade... Done
The following package was automatically installed and is no longer required:
libevdev2
Use 'apt-get autoremove' to remove it.
The following packages have been kept back:
initramfs-tools linux-generic-lts-vivid linux-headers-generic-lts-vivid
linux-image-generic-lts-vivid
0 upgraded, 0 newly installed, 0 to remove and 4 not upgraded.
