If you are using apt-proxy this line is probably not correct :
FAI_DEBOOTSTRAP="$debdist file:/home/apt-proxy/mi/ $debdist main"

apt-proxy is an http proxy so your line should be :
FAI_DEBOOTSTRAP="$debdist http:://localhost:9999/mi/ $debdist main"

Where localhost is the server where apt-proxy is configured and 9999 the
port give in
apt-proxy.conf
