LVM_RAID

Hi guys,

I would like to propose a patch for the SW_RAID_LVM hook [0]. This hook can
currently only do LVM on top of a mdadm software RAID. I think it could be
interesting to have a single hook that would allow :
- LVM over SW RAID
- LVM without SW RAID
- SW RAID without LVM
- SW RAID and LVM (the LVM not using the SW RAID)

The attached patch allows this by simply not dying when the RAID variable is
not set. It has been tested and I hope it can be useful :)

I also had an issue with mdadm crashing when trying to create volumes. The
quick fix I found was to do a stop/remove before creating the volume. This
part is also included in the patch. I'm sure there is a cleaner way to fix
it, if anyone is wanting to throw an eye on it.
